Authorization to Release Information - This form can be used to authorize ECSI and the Student Loan Service Center to release information to a third party, such as a parent or spouse.

Name Change Form -  Requests for name changes require a copy of government issued identification, a copy of an updated social security card and legal documenation confirming the reason for the name change.

Nurse Faculty Loan Cancellation Request Form - This form can be used to request a cancellation on a Nurse Faculty Loan. 

Forbearance for Economic Hardship or Unemployment - Use this form to request a forbearance on non-Perkins Loans if you are experiencing financial difficulties and cannot afford payments. 

Nursing or Medical School Loan Deferment-Forbearance for In-School Deferment -  You may submit this form to request deferment while you are enrolled in an eligible college or career school.

Medical Residency Deferment-Forbearance Request - Use this form to request a payment deferment if you are a medical doctor completing a medical residency.

Perkins Loan Specific Forms

 

Perkins Cancellation Form - Submit this form to request postponement (deferment) and/or cancelation on a Perkins Loan.  Must submit annually or if changing jobs. 

Economic Hardship Deferment Request - Use this form to request a deferment while you are experiencing financial difficulties or serving in the Peace Corps.  May request for up to three years.

Unemployment Deferment Request - Use this form if you are unemployed or unable to find full-time employment.  May request for up to three year.

In-School Deferment Request - You may submit this form to request deferment while you are enrolled at least half-time at an eligible college or career school.

Military Service and Post-Active Duty Student Deferment Request - You may use this form to request deferment while you are on active duty military service in connection with a war, military operation, or national emergency or if you were on active duty military service in connection with a war, military operation, or national emergency, for the 13 month period following the conclusion of that service, or until you return to college or career school on at least a half-time basis, whichever is earlier.

Rehabilitation Training Program Deferment Request - This form can be used to request deferment while you are enrolled in an approved rehabilitation training program for the disabled.

Cancer Treatment Deferment Request - You may use this form to request deferment while you are receiving cancer treatment and for six months following the conclusion of your treatment.

Graduate Fellowship Deferment Request - You may use this form to request deferment while you are enrolled in an approved graduate fellowship program.
